Expanding Access to Inclusive Wellness: Adapt & Thrive, LLC Now a Provider with Lakeland Care and CLTS Programs

We’re excited to share that Adapt & Thrive, LLC is now an approved provider for individuals served through Lakeland Care and the Children’s Long-Term Support (CLTS) Program in Wisconsin! Brown and all surrounding counties are able to be served!

Based in Green Bay, Adapt & Thrive was founded with a clear mission: to create inclusive and accessible health and fitness opportunities for individuals with physical, intellectual, developmental, and neurological disabilities. We specialize in bridging the gap between physical therapy and the commercial gym setting by offering customized, adaptive wellness programs designed for long-term success.

• Goal-oriented wellness planning

By becoming a provider through Lakeland Care and CLTS, families and individuals can now access our services using waiver funding, eliminating financial barriers and expanding access to consistent, supportive programming.
